Titration of a medicine means slowly increasing the dose of a medicine by very small amounts over days, weeks, or even months, to find the right dose that is effective for you.

The RADS assessment and report indicated Molly’s Lorazepam should be titrated within 4-7 days of the new regime (two weeks tomorrow) and not to be used as a PRN, this didn’t happen and there was much clarification needed. I called the team in Carlisle to query on Tuesday of this week, they needed to see Molly before a decision to titrate could commence. A home visit took place and in summary they wanted to wait and see, the rationale was that they were taking her off venlafaxine and introducing lamotrigine. I was informed by the second opinion that we had to treat the Catatonia aggressively yet we now have to wait?
So today I supported Molly who has remained in bed for sometime now, the morning was challenging with lots of negativism – This means a person doesn’t react to something happening around them or actively resists what’s happening around them for no rational reason. Exactly the experience I witnessed today.
I asked for a PRN of lorazepam and the magic happened, she accepted a bath, had her hair washed, teeth cleaned and then asked to get dressed. Normally she goes back to bed, we had lunch, went out to have coffee and cake and even managed a short dog walk. Lorazepam affects wear off and I could see she probably needed a top up, I wouldn’t dare of asked but I will however report my findings tomorrow to the Carlisle team.
Catatonia is treated by giving Lorazepam and the plan was to have it titrated up to between 6mg and 8mg’s per day.
